Under the new General Data Protection Regulations I need to tell you what we do with the personal information that you have supplied to me.
Life Fusion is committed to protecting your privacy and ensuring the security of your personal data.
If you contact me through my website or by email, I will reply to you by the email address you supplied to me. If you have given me your business card or supplied your contact details at a workshop, I will also contact you via your email address. If you contact me directly by phone, then I will contact you by phone in return.
Storing your Personal Information
All personal information provided to me is currently stored in an Excel spreadsheet. This data held is your name, address, phone numbers, email address and the reason you contacted me.
In the future, this data may be transferred to the Mailchimp newsletter service for ease of informing you of any changes to my services.
Your personal data is not used for any automated decision making or profiling.
Once you have become a client, all notes taken pertinent to your coaching, counselling or nutritional therapy sessions are recorded in paper format only.
To meet legal obligations, your personal data and notes will be stored for the duration of you being an active client and then for seven years after your last appointment. All records relating to a minor will be retained until the child reaches the age of 25.
After this time all paper records are destroyed, but your personal details are retained on the Excel spreadsheet.
Life Fusion will keep your personal information safe and secure. It is only held on my personal computer or in my office. I am the only person who has access to it.
Life Fusion will not disclose your personal information unless compelled to, in order to meet legal obligations, regulations or valid governmental request.
Your Legal Rights
You have a number legal rights over your personal data held by me. These include:
- Right of access: You have the right to request a copy of the information I hold on you
- Right of rectification: You have the right to correct incomplete, inaccurate or outdated personal data
- Right to be forgotten – You can request your data is erased from my records, subject to the legal requirements mentioned above
- Right of portability: You have the right to have the data I hold on you transferred to you or another organization
